Career summary
A physiotherapist helps people move, breathe and use their bodies better. Your patients might be a rugby player with a torn knee, a grandmother learning to walk after a hip operation, a baby with weak muscles, or a patient on a breathing machine in ICU. You work out what is wrong with how they move, then plan treatment to fix it.
Most of the work is hands-on. You guide exercises, move stiff joints, help patients clear their lungs and teach them how to look after themselves at home. Because patients only see you for a short time, a big part of the job is explaining. If they don't understand their exercises, they won't do them.
To become a physiotherapist in South Africa you need a four-year university degree in physiotherapy. You must register with the Health Professions Council of South Africa from your first year as a student. After graduating you do one year of paid community service, usually in a public hospital or clinic, before you can work independently.
A Day in the Life
This is a typical day for a physiotherapist in a South African public hospital, such as someone doing their community-service year. A private-practice day looks different: more booked appointments, fewer ICU patients.

Check the lists — You read the handover lists for the ICU and wards. You decide which patients need you first, usually the ones struggling to breathe or who had surgery yesterday.

ICU chest physio — In ICU you help very sick patients clear their lungs, change position and, when they are ready, sit up or stand for the first time. Some are on breathing machines. You work closely with nurses and doctors.

Ward round — On the orthopaedic and surgical wards you get patients walking after operations, teach them to use crutches safely and give them exercises they can do in bed.

Outpatients — People come in with back pain, neck pain, healing fractures or weakness after a stroke. If you don't share a language, a nurse, colleague or family member may help you explain the exercises.

Lunch and notes — Every treatment must be written up in the patient's file. You also record the day's numbers for the department.

Group class or clinic — You might run a back-care class, a neuro gym for stroke patients, or a clinic for children who are slow to sit, crawl or walk.

Community visits — In rural posts you may visit homes or clinics. Some patients can only get to you every few weeks, so you write clear home programmes that family members can help with.

Last ICU round — You check the ICU patients again and hand over. On some days you are also the physio on call for ICU after hours or over the weekend.

Registers you as a Physiotherapist
The only South African qualification that leads to registration as a Physiotherapist, offered at public universities. Each university names and registers its own version with SAQA, so there is no single SAQA ID.

Physiotherapy degrees take small groups each year and selection is competitive. Meeting the minimum marks does not guarantee a place, so apply to more than one university.

Moving up a pay grade in government takes years
In the public service, a physiotherapist needs 10 years' experience after registration to be paid at Grade 2, and 20 years for Grade 3.

- Mathematics keeps you eligible at every university we checked, as long as you also have the other subjects they ask for.
- You meet the Mathematics requirement most universities set for physiotherapy.
- You are not relying on getting a very high Maths Literacy mark.
You take Mathematical Literacy
- Most physiotherapy degrees close. Of seven universities we checked, six ask for Mathematics.
- One university's physiotherapy page accepts Maths Literacy, but only at Level 6 (70% or more), and it also wants Physical Sciences and Life Sciences (2025 entry requirements).
- That leaves you with one realistic option instead of several, and physio places are already very limited.
Best School Subjects
Strongly recommended
- Mathematics · Almost every physio degree we checked requires it. Only one accepted Maths Literacy, at a very high mark.
- Physical Sciences · Most physio degrees we checked require it; a few accept Life Sciences instead. Taking both keeps every door open.
- Life Sciences · Some universities require it too, and it helps with anatomy and physiology.
- English Home Language · English is a listed entry subject at most universities checked (Home or First Additional Language).
Professional Registration
Registration with the Health Professions Council of South Africa (HPCSA) is compulsory to practise. Under the Health Professions Act 56 of 1974 (s15 boards; s17 registration required to practise; s18-19 registers/removal; s39 offence to practise unregistered).

The rules for physio are being updated
The regulations that describe what a physiotherapist may do date from 1976. On 18 September 2026 new draft regulations were published for public comment. Until they are final, the 1976 rules still apply.

Applying to register in the category Physiotherapist with a qualification from outside South Africa. If you study physiotherapy in another country, you must pass a theory exam and a clinical exam set by the HPCSA before you can register. South African graduates of accredited degrees do not write this exam.
Community service: 1 year, paidCommunity service
Registering for the first time in the category Physiotherapist, including physiotherapists who qualified outside South Africa. During the year you are on the register of persons performing community service. After you graduate, you work for one year in a public hospital or clinic chosen from an approved list, often in an under-served area. It is a paid job under public-service conditions. You must finish it within two years. Only after this can you register to work independently, including in private practice.

Not a register-category duty. It is part of the accredited degree that leads to the category Physiotherapist; while studying you must be on the HPCSA student register. During your degree you must do at least 1,000 hours of clinical training with real patients, in hospitals, clinics and other settings. You must also stay registered with the HPCSA as a student from first year until you finish.
Your first job could be far from home
The community-service year is compulsory and you are placed in an approved public facility, often in a rural area. Plan for living away from home for that year.
